A day in the life of a patient
2.30pm
Alan, a 72-year-old male presents to the emergency department with a CHF exacerbation. The ED physician determines that he is a candidate for Hospital at Home, based on an alert from pre-defined criteria in the EHR.
A hospital-at-home nurse brings a Current Health kit to the ED, consents the patient, and admits him to the Current Health platform.
6.00pm
Alan receives a welcome call from Current Health to ensure he is successfully set up. He then uses the tablet provided to complete his eDiary, reporting that he is home, feeling well, and has taken his medication.
He continues to receive prompts and questions on the tablet each day to encourage adherence with his care plan.
8.00am
The hospital-at-home team uses the Current Health dashboard to round on patients for the day. Within the dashboard, patients are grouped by risk level.
The team schedules Alans in-home visits and he receives a notification for when to expect a nurse.
9.45am
A member of the in-home care team visits Alan twice each day to confirm readings, reconcile medication, and disuss his care plan.
The nurse logs notes within the Current Health application, which syncs with the EHR.
4.30pm
Alan’s vitals begin to deteriorate over the course of an hour, including a drop in oxygen saturation and an elevation in respiratory rate. He reports feeling breathless.
These vitals meet the threshold of the alarms configured for a CHF patient.
4.45pm
The monitoring detects a deterioration and alerts the Current Health Clinical Command Center nurse.
The nurse initiates a video call with Alan and then escalates to the hospital-at-home care team member, who increases his medication dose.
9.00am
The hospital-at-home team continues to monitor Alan through the platform. Since increasing his medication, Alan has been feeling well. He reports feeling safe and secure in his home knowing he is connected to his care team 24/7.
After 10 days, Current Health collects the kit from Alan’s doorstep, reprocesses it, and delivers it back to the hospital for the next patient.
